A milking table does one thing exceptionally well: it puts someone face-down on a padded surface with their relevant 'bodypart' accessible through a hole below. What happens after that is a conversation between you and your partner. But the specific positions you use, where the giver is relative to the table, how the receiver is oriented, and what kind of access you're going for all make a genuine difference to how the experience plays out.

How a Milking Table Creates Access

The geometry is simple: the receiver lies face-down on the table. Their anatomy drops through the hole. The giver positions themselves below or beside the table. The table surface is padded and raised off the floor, so the giver doesn't have to lie flat on the ground to access the hole from below. Everyone is comfortable, no one is at an awkward angle, and the whole setup takes under five minutes.

What changes between positions is how the giver is positioned relative to that hole, which changes the angle and type of access. Lying directly below gives one angle. Kneeling gives another. Sitting on a chair or cushion gives more stability for longer sessions. Side access opens up manual stimulation options that below-access doesn't offer as easily. There's more variation here than most people realise going in.

The Milking Table Positions

Position 1: The Classic (Below, Lying or Semi-Reclined)

This is the position the table was designed around. The giver lies on their back or semi-reclined below the table, looking up at the hole. The receiver lies face-down above, fully relaxed, with their anatomy through the opening. The giver has hands-free oral access, full manual access, or both.

It works because both people are comfortable and stable. The receiver isn't supporting any of their own weight or doing anything active. The giver isn't craning their neck or holding a difficult position. The table does all the structural work, which is the whole point.

Best for: Oral stimulation, prostate massage, extended sessions where both partners want to relax into it. Works with all table models but particularly well with the open-underside MIDNIGHT and PRO.

Position 2: Kneeling Below

The giver kneels below the hole rather than lying down. This raises their head position, changes the angle of access, and gives them more mobility and control over movement. It's easier to switch between oral and manual stimulation from a kneeling position than from lying flat.

The trade-off is knee comfort over longer sessions. A thick cushion, a folded blanket, or a dedicated kneeling pad makes a meaningful difference. This position works best at a table height where the hole is roughly at the giver's face level when kneeling. For most people that's 24 to 27 inches, which is why adjustable height is useful if there's a significant size difference between partners. 

Best for: Couples who want the giver to have more active control, mixed oral and manual play, shorter to medium-length sessions.

Position 3: Seated Below (Chair or Low Stool)

The giver sits on a low chair, stool, or firm cushion below the table. This is the most ergonomically sustainable position for longer sessions. The giver's back is supported, their arms are in a natural position for extended manual stimulation, and there's none of the knee strain from kneeling. It's also the easiest position to maintain consistently, which matters when consistency matters to the receiver.

Height matching is important here. A seat that puts the giver's hands or mouth at the right height relative to the hole will vary based on the height of the chair and the height of the table. Experiment before committing to a session. The MILKER: PRO is the winner here.

Position 4: Side Access

Instead of positioning below the table, the giver kneels or sits beside it, reaching through the opening from the side. This gives a completely different angle for manual stimulation and changes the experience in ways that can be surprisingly good. The receiver often finds the change in angle and motion more intense than straight-below access.

Side access works especially well with a rectangular hole (the format on the MIDNIGHT, MOO, and PRO) because there's room to move hands in from the side without the circular edge of a round hole getting in the way. With a round hole, side access is noticeably more restricted.

Best for: Manual stimulation, varying the experience mid-session, couples who want to mix up position without stopping.

Position 5: Crouching (Tall Tables Only)

With the MILKER PRO at 37.5 inches, the giver can access the hole from a standing or slightly crouching position. This is a genuinely different experience for both partners. The giver has more reach, more range of motion with their arms, and a completely different dynamic. It's the position that most closely mirrors standing play with no table at all, except the receiver is fully supported and in a fixed position.

Positions for Women on the Milking Table

A milking table works for women in essentially the same structural way: receiver lies face-down, partner accesses from below or from the side. The critical requirement is a large rectangular hole rather than a small round one. A 4 to 5 inch round hole is not big enough for comfortable female use in most positions. Receiver Face-Down, Oral Access

The receiver lies face-down with their hips positioned over the hole. The giver lies below and provides oral stimulation. This is one of the more intimate setups the table enables. The receiver is completely passive and fully supported, which creates a different kind of relaxation than most positions where at least some weight is being held. The angle is different from any floor or bed-based equivalent.

Receiver Face-Down, Manual Access

Same positioning, with the giver using hands from below or from the side. Works well for clitoral stimulation with the giver below, or penetrative play from beneath. Side-access manual stimulation in particular tends to work very naturally with the rectangular opening.

Advanced Positions and Restraint Play

Restrained Face-Down

The receiver lies face-down on the table with wrists tied to the front table legs and ankles to the rear legs. They have no mobility, no control over access, and nowhere to go. For couples who enjoy power exchange dynamics, this is one of the table's best features. The person on top is genuinely restrained while remaining comfortable on a padded surface. The setup is fast and the restraint is firm without requiring complex rigging.

Standard under-the-bed restraint straps work well for this. Tie wraps or cuffs around table legs also work if you own a set. The MILKER PRO's taller, heavier frame handles restraint better than the lighter CLASSIC.

Blindfolded and Restrained

Adding a blindfold to the restrained position takes the sensory experience somewhere most couples don't access through regular play. The receiver has no visual information, no control, and is fully dependent on the giver.
